Please use this form to request items that are firm ordered. Specifically, these would be books, VHS/DVD’s, microforms, serial back issues, single e-books, software, and scores.

The form has been revised to accommodate changes selectors requested, specifically, the option to provide an OCLC record number. If a title and an OCLC record number are provided, MAS will overlay the brief data with the OCLC record as the item is entered into CLIO. If an OCLC record number is not provided when completing the form, the bibliographic data entered will form the basis of the CLIO record. If the data provided is insufficient, the order will be referred back to the selector.
