It is really important to have the person experiencing the problem fill out the form. The form gathers information in the background about permissions, IP addresses, and software. CERM needs these to diagnose problems.
The form also goes to an alias that is monitored. If you send an email to an individual, and that person is not in the office, your problem will sit until the person returns.
